Widal test is a tube agglutination test employed in the serological diagnosis of enteric fever. Typhoid fever is a systemic infectious disease characterized by an acute illness, the first typical manifestations of which are fever, headache, abdominal pain, relative bradycardia, splenomegaly, and leucopenia. The diagnosis of typhoid fever on clinical grounds is difficult, as the presenting symptoms are diverse and similar to those observed with other febrile illnesses. The definitive diagnosis of typhoid fever requires the isolation of Salmonella enterica serotype Typhi from the patient. This test looks for the presence of agglutinins to the O and H antigens carried by S. typhi and S. paratyphi A, B and C, which are responsible for typhoid and paratyphoid fever. HIGH anti H and anti O antibody titres are suggestive of a positive diagnosis of typhoid fever.
