Acylcarnitine is produced from the amino acid carnitine by the enzyme carnitine acyltransferase, and can have differing chain-length specificities, depending on the cellular location and metabolic purpose. Carnitine and its acyl esters (acylcarnitine) are essential compounds for the metabolism of fatty acids, as they assist in the transport of fatty acids to the mitochondria where they are oxidized as a major source of energy. Acylcarnitine test can assist in the diagnosis of any muscle fat metabolism disorder. HIGH levels are associated with fatty acid oxidation disorders and organic acidurias.
